Main-language correctness epic.

A missing lookup result is not itself an error. The attached sub-issues first make resolution states explicit, then publish only diagnostics justified by one document/index snapshot, then offer actions from structured diagnostic data.

The model must distinguish a missing target from an unavailable graph, unfinished indexing, an unfetched repository, an unknown repository mapping, a visibility or kind mismatch, and a target pattern. Wrong is worse than absent.
